The Hospice Team
Interim HealthCare Hospice provides a compassionate, patient-centered approach delivered by a team of highly qualified professionals who share a commitment to meet the physical, emotional and spiritual needs of patients and those who love and care for them. If care takes place at home, family members provide the day-to-day care and the hospice team supports the family in providing end-of-life care. Care can also be provided in nursing homes, senior living residences, including independent and assisted living facilities.

What is Hospice Care?
Hospice care provides comfort care, symptom management, emotional, and spiritual support to patients nearing the end of life. Hospice focuses on quality of life and helps patients live their last days with a sense of control. Interim HealthCare Hospice is available to answer any questions patients and their families may have to assist in choosing a hospice agency during this difficult time.
Where is Hospice Provided
Hospice is a method of care, not a physical location, and is provided anywhere a patient my call home. It may be offered in the person's home, a nursing home, a designated hospice facility or any other facility.
Who Pays for Hospice?
Medicare, Medi-Cal, and most private insurance plans cover hospice care. Families may be asked to meet some costs that aren't covered by insurance. Public and community support make it possible for hospices to cover the cost of care. This happens through donations and grants.
Traditionally, hospice is an option for people whose life expectancy is six months or less, and involves palliative care. Medicare is the primary source of payment for hospice. Care focuses on pain and symptom relief, rather than ongoing curative measures. Some hospitals, nursing homes, and other health care facilities provide hospice care onsite, however, in most cases hospice is provided in the patient’s own home which provides a familiar and comfortable environment.

Alzheimer's disease is a painful condition for seniors and their loved ones to face. A person living with Alzheimer's faces deteriorating mental and physical health, which can escalate to the point where one can no longer recognize or remember their nearest and dearest loved ones.
